Inquest heard man died after being discharged from unit

A JURY has been hearing evidence about a man who was found dead at his home after being discharged from a mental health unit.

Patrick Whiting, aged 43, was found dead in Brighton in May last year.

An inquest at Hove Crown Court heard last week he had been receiving treatment at the Woodlands Centre for Acute Care in St Leonards.

The inquest started last Monday and was due to continue today.

A number of changes have been made following Mr Whiting’s death.

A statement by Dr Shakil Malik, clinical director for adult mental health services at Sussex Partnership NHS Trust, said: “We regret that our usual high standards were not always followed in Mr Whiting’s care. Errors were made. We have apologised unreservedly to Mr Whiting’s family. Following Mr Whiting’s death, we looked in detail at what happened.

“We have made some changes to the way we assess risk, share information and support patients and their families when they are discharged and receive care at home.”
